Question
My breast became to sag after childbirth of over 4 children due to weight loss and childbirth, I want to restore them through a breast lift, is this permissible?

Alhamdulillah, wassalatu wassalamu ala rasulillah, wa ala alihi wa sahbihi ajmain.

1. The Shari Context

Islam distinguishes between treatment and correction versus changing the natural creation for beautification.

Allah says:

“And I will command them, and they will alter the creation of Allah.”
Surat al Nisa 4:119

At the same time, Islam permits medical intervention when there is a true need, such as correcting harm, injury, or clear deformity.

The Prophet ﷺ allowed corrective procedures in cases of injury and abnormality, which scholars use as a basis for permitting restorative treatment, not cosmetic enhancement.

2. Scholarly Discussion

The scholars differentiate between:

  • Corrective procedures due to:
    • Injury
    • Deformity
    • Abnormal physical condition
  • Cosmetic procedures done for:
    • Increasing beauty
    • Reversing natural aging
    • Enhancing appearance beyond the normal state

They generally allow the first category and prohibit the second.

Changes resulting from:

  • Aging
  • Pregnancy
  • Weight fluctuation

are considered natural and expected, not deformities in the legal sense.

Because of this, many scholars do not permit surgical intervention purely to reverse such natural changes, especially when it involves:

  • Surgical exposure of awrah
  • Medical risk
  • Pursuit of enhanced appearance

3. Application to the Question

In your case:

  • The change is due to pregnancy and natural bodily processes
  • It is not the result of an accident or abnormal deformity
  • It falls under natural change over time

Therefore:

  • A breast lift for the purpose of restoring appearance in this case is not permissible
  • It is treated as cosmetic enhancement, not necessary treatment

However, if there were:

  • A clear medical issue
  • Or a severe deformity outside the normal range

then the ruling could differ.

4. Relevant Usul Principle

Blocking the means to harm

This principle means that actions which may open the door to impermissible outcomes are restricted. Allowing cosmetic alteration of natural body changes for beautification leads to broader normalization of changing Allah’s creation without necessity, and opens the door to excess, imitation, and harm.

Final Ruling

A breast lift due to natural changes from childbirth, aging, or weight loss is not permissible, as it falls under cosmetic alteration rather than necessary correction. Only in cases of clear deformity or medical need would such a procedure be allowed.
